New Patient Registration

register_now_pink_signWhen registering with Drumragh Family Practice you will be registering with the Practice and not an individual GP. For administrative purposes your medical card will be issued in the name of one of the doctors; however, you can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by- case basis. This preference will be recorded in your computer record and we will do our best to respect your choice. However, not all the doctors in the practice provide every service and specific doctors may not be available immediately.

When registering, please bring your NHS Medical Card to the practice and those of any other persons registering with you. If you do not have a medical card, you can either call into the surgery and collect a registration form, or alternatively you can download one from the link on the home screen.  

Persons applying for National Health Service registration for the first time are obliged to present their passport or work permit which we will photocopy and send with HS22X (please ensure that your download this form if doing so online). 

Alternatively if you have been registered within the NHS before please use the HS200 form (also available on the link).

 The practice also requires a recent utility bill and photographic I.D.  Children under the age of 7 years will not be accepted without a copy of Childhood Immunisations/Vaccinations. Your medical card/form will be forwarded to Central Services Agency who will issue you with a new Medical Card. This takes approximately 6 weeks.

If you move out of the practice area, you will need to register with a new practice as soon as you move. We can provide information about how to find a new practice. Patients registered with a local practice will be required to make an appointment with the Doctor who will then decide whether the practice will accept you. If the Practice does not wish to accept you and you have difficulty in getting registered you should contact:
